- Eval Type: Pulls information from the type of Evaluation Event
- A-1: Pulls information from the Date Received on the A-1 -Referral for Special Education Evaluation
- A-2/A-2S: Pulls information from the A-2 - Notice of Receipt of Referral or Notice of Reevaluation>Form Date or the A-2-S - Notice of Agreement that a Three-Year Reevaluation Not Needed>On date
- A-3 Sent: Pulls information from the A-3 - Notice and Consent Regarding Need to Conduct Additional Assessments>Form date
- A-3 Received: Pulls information from the A-3 - Notice and Consent Regarding Need to Conduct Additional Assessments>Date Signed Consent Received or Denied.
